Fall Sports Physicals and PaperworkInformation – 2019/2020 School Year

According to P.I.A.A. rules, a student must have a physical authorized AFTER  June 1, 2019 in order to be eligible to participate in inter-scholastic sports for the 2019-2020 school year. Every student-athlete, grades 7-12, is responsible for completing the following paperwork:

  1. P.I.A.A. paperwork (CIPPE Form Sections 1-6).
  2. Notre Dame High School paperwork (ND Athletic Forms).

Both sets of paperwork can be found in the main office and/or on the Notre Dame High School website: www.ndcrusaders.org under the Athletics tab, labeled ‘Athletic Department’

As far as the processing of the physicals and other necessary paperwork, we have designated (2) dates this year to collect the paperwork:

OPTION 1: Doctors from St. Luke’s Orthopedics will be at Notre Dame to perform a sports physical.

When: Wednesday, June 12th from 5:00 – 7:00 pm in the Notre Dame Main Gym

Cost: NO CHARGE

Both sets of paperwork NEED to be handed in at the time of the physical (please see above)

OR

OPTION 2: Monday, August 5, 2019 – 9:00 am to 2:00 pm in the Athletic Office

**Only paperwork collection will be on this date.  Students must have had a physical from their own physician. Notre Dame paperwork must also be turned in as well.**

 

Fall Practice Start Date

Fall sports practices begin on Monday, August 12, 2019. Only those students who have completed and turned in their paperwork to the Athletic Office will be allowed to begin.

Once a student-athlete receives a physical exam after June 1, 2019, it is good for the entire

2019-2020 school year, in any sport they choose to participate in. Also, once the Notre Dame documents have been properly completed and handed in, they are good for the entire 2019-2020 school year as well.

PLEASE NOTE – No paperwork will be accepted until the day and time assigned and must be submitted in person to the Athletic Director. Please do not attempt to hand any paperwork to the Main Office Staff.  They have been instructed NOT to accept any paperwork. The Notre Dame High School Athletic Department assumes no responsibility for any paperwork handed in, faxed, mailed, emailed, or in any other way, transmitted to Notre Dame High School prior to the assigned date and time for processing Fall sports candidates for the 2019-2020 season as mentioned above.
