Dear Parents/Guardians,

Back to School Reminder The Pennsylvania Department of Health School Code and the State Board of Nursing in Pennsylvania mandate a Current Physical Exam and Immunization before the start of the school year.  PA DOH also mandates a second Meningitis vaccine for 12th grade.  PA State School Exclusion deadline is October 1st.

All students entering 9th &11th grade MUST have a copy of the Current Physical exam and Immunizations completed and sent to the Nurse’s Office within the first weeks of School (October 1st  deadline) for School Health Records. 

All students entering 12th grade MUST receive a second MCV4 (Meningitis) and a copy of vaccination records must be sent to the Nurse’s Office within the first weeks of School (October 1st deadline) for School Health Record.  

Bethlehem Area School District and Notre Dame High School policy for students who require care or medication in school for routine or emergent conditions must complete required Medical Forms and Student Health Plan.  All Completed forms and Medications are to be returned to ND Health Services.  All forms are located on the Notre Dame website in the Nurse or Student Health and Safety tabs. 

Medical Forms and Student Health Plans

Please complete and submit the required forms and any Student Health Plans before the start of the school year.  These forms include:  

Medication Administration Guidelines 

Over The Counter OTC Medications

  •  BASD OTC Medication-Treatment Parent Consent form must be signed and returned to school. Please note this authorization is valid for the current school year.
  • Medication will be given more than two times during the school day without a Health Care Provider Order.
  • The nurse reserves the right to refuse to dispense medication at any time based on the assessment of the situation and every effort will be made to notify the parents of the situation (ie. repeated requests or overuse).

Prescription Medications 

  • Medications will only be administered with a Health Care Provider’s Order and the signed Medication Authorization.  Please use a separate form for each medication.
  • All Medications must be delivered to the Health Services/Main Office, by parent or guardian in unopened original packaging or prescribed container.
  • Rescue Inhalers and Epinephrine are the only allowed self-carry medications, with a Health Care Provider’s Order and signed Asthma Health Plan or Anaphylaxis Health Plan.

Copies of the care plan will be given to teachers, bus drivers, physical education instructors, and cafeteria personnel on a “need-to-know” basis.  A need to share information would be considered for such situations and safety as severe bee sting or food allergies, seizure disorders, insulin-dependent diabetes, heart conditions, and unstable asthma.  If there is information that you would like the medical staff to know, and it is not specifically addressed on the form, please document it accordingly.  You may contact Health Services anytime with your questions or concerns.  We appreciate your taking the time to review this information, and providing the school with your student’s Health Updates so we may provide quality care to your student.
